Indications- Bronchospasms associated with emphysema, asthma, bronchitis, and anaphylaxis Contraindications- Hypersensitivity, angioedema, cardiac dysrhythmias
Step 1- Repeat the drug order to ensure proper dosage for patient.
Step 2-Begin to explain the procedure to the patient ex. “Your doctor has ordered some albuterol via nebulizer pump at an attempt to help you breathe better, have you taken albuterol via nebulizer before? (Wait for patient’s response) This medication will not hurt only help you. Verify that the patient does not have any allergies to the prescribed medication. Check the patient’s base line vitals, and then connect the patient to EKG monitor for constant monitoring. At this point consider evaluating Peak Expiratory Flow (only if the device is available).
Step 3- Examine the medication thoroughly. While examining the medication one must check for certain key elements that can alter the drug in question and cause harm to the patient who is to receive the drug. Check the name of the drug from the label on the medication to make sure that it is the correct medication prescribed for the patient. Then check the medication for its vessel integrity, color, clarity, concentration, any sedimentation, the expiration date, also check the total volume of medication and total milliliters of medication.
Step 4- Verify once AGAIN that the patient has no allergies to the medication you are preparing.
Step 5- Expose the medication chamber by opening the nebulizer.
Step 6- Twist off the top of the medication and pour the desired amount of medication into the medication chamber.
Step 7- Close the medication chamber ensuring that the cap is on tight so no medication will spill then attach all tubing and mouth piece or facemask.
Step 8- It is now time to connect the nebulizer to the tubing that supplies oxygen to the machine. Machines vary, be sure to check manufacturer’s recommendations. (Commonly 6 l/m or 8 l/m)
Step 9- Attach the mouthpiece or face mask to the patients face and properly secure. Encourage the patient to take deep breaths so that the midst created from nebulizer reaches the desired location (lungs) ensuring maximum benefit.
Step 10- Reassess the patient. While observing the patient look for undesirable effect as well as desired effects.
Step11- Update medical control as desired or required and record all changes.
